Central Coast Collaborative Pathway Project

To promote and support quality End of Life care for all residents of the Central Coast regardless of age, disease or care setting.
Introduction

Caring for the dying occurs in every care setting on the coast. Differences in the availability of resources, experienced staff and expertise in this area means that there is significant variation in what can be provided for clients in different care settings during this important time.

The Central Coast Palliative Care Service has designed a package to assist care settings in achieving the dual goals of providing and documenting quality end of life care. Partners in this ongoing project will be provided with resources (including a generic End of Life pathway, implementation guidelines, educational packages, web based materials) backed up by specialist input as they develop processes for End of Life care tailored to suit their individual setting.

The partnership program also includes a quality mechanism designed to allow project partners and the CCPCS to continually refine and improve the pathway performance and outcomes .

The CCPCS would like to invite Hospitals (private and public), Residential Aged Care Facilities, and General Practitioners on the Central Coast and other Area Health Services to become partners in this project.

What is provided

  1. Pathway templates in electronic and paper form
  2. General Implementation Guidelines
  3. Staff Education package
  4. Specialist support for pathway adaption, implementation and staff education and training.
  5. Web based materials and support site access
  6. Tri-annual Pathway Partnership Quality and Development Meetings
  7. Collation and reporting of quality data to Tri-annual Pathway Partnership meeting
  8. Provision of pre and post implementation assesment tools ( if required for individual site quality projects)
  9. Professional support 24/7

Partnership requirements

Hospitals(Private & Public) and Residential Aged Care Facilities

  1. Specified representation of a Senior Manager for each participating care site.
  2. Commitment to ongoing quality development through problem reporting and participation in Tri-annual partnership meetings

General Practitioners

The pathway is easily adapted for use in the patients home. A joint project between the CCPCS and the Division of General practice is in place and GPs or practice managers should contact the Division in the first instance.

Cost

The service is provided free of charge by Central Coast Palliative Care Service .
