RCA Causal Statement Meta-Analysis
Northern Sydney Central Coast Area Health Service (NSCCAHS) provides high quality care to thousands of patients every year. In a small number of cases, adverse incidents do occur.
As part of our commitment to quality care, Root Cause Analysis investigations are undertaken into these significant incidents to help clinicians learn from adverse events and improve health services provided to our community. Underlying trends from RCAs are analysed so that the whole system can learn.
In addition to RCA reports from NSCCAHS, there is also a link to the Clinical Excellence Commission (CEC). The CEC was established in August 2004 as part of the NSW Patient Safety and Clinical Quality Program to ensure that patient safety and excellence in clinical care remain top priorities for the NSW Health system.
